Service Contract

Life Mates Co., Ltd. (hereinafter referred to as "Life Mates") and "____________________" (hereinafter referred to as "Independent Party") enter into a Service Contract (hereinafter referred to as "this contract").

Article 1 (consignment work)

Life Mates entrusts the following business (hereinafter referred to as "this business") to Independent Party.

1. Matching service for marriage partner introduction service

2. Business related to the above

Article 2 (consignment fee)

1. Life Mates will pay the commission fee separately specified between Life Mates and Independent Party for this business to Independent Party by the payment deadline and payment method separately determined by Life Mates and Independent Party.

2. The cost required to carry out this business shall be borne by Independent Party.

Article 3 (contract period)

1.The term of this contract is from year___________month___________day___________to year___________month___________day___________. If there is no written cancellation request from Life Mates or Independent Party by one month before the expiration of the term, one year will be automatically set under the same conditions. It will be extended and so on.

2. Life Mates may cancel this contract at any time by notifying Independent Party in writing one month in advance.

Article 4 (Prohibition of subcontracting)

Independent Party shall not contract all or part of this work to a third party without the prior written permission of Life Mates.

Article 5 (Confidentiality)

Independent Party shall not leak the information obtained regarding this business to a third party during or after the expiration of this contract. If Independent Party’s violation of this article is discovered, Independent Party must pay Life Mates 5 million yen as a violation.

Article 6 (Prohibition of competition)

During the term of this contract and for 5 years after the termination of this contract, Independent Party shall not engage in any business that competes with this business, either by Independent Party self or a third party, without obtaining the prior written consent of Life Mates. If Independent Party’s violation of this article is discovered, Independent Party must pay Life Mates 5 million yen as a violation.

Article 7 (Prohibition of transactions with antisocial forces)

Life Mates and Independent Party shall not engage in transactions with members of anti-social forces or persons equivalent thereto.

Article 8 (Agreed Jurisdiction)

In the event of a dispute regarding this contract, the summary court or district court having jurisdiction over the head office location of Life Mates shall be the exclusive agreement competent court of the first instance.

Article 9 (Consultation)

If there is any matter not stipulated in this contract or if any doubt arises regarding this contract, it shall be decided after consultation between Life Mates and Independent Party.

Article 10 (Special Contract)

Life Mates and Independent Party confirm that the relationship between Life Mates and Independent Party is outsourcing and not an employment relationship. Independent Party has to pay for health insurance and taxes by him/herself.
